Monument of Faith Church of God in Christ and Bishop Herron in conjunction with Trumbull County Combined Health District (TCCHD) will be hosting COVID-19 Vaccine clinics at the Ohio Central East Ecclesiastical Jurisdiction Holy Convocation Week. The vaccine clinic will occur daily from August 9th-13th from 6:00pm – 8:00pm at Warren First Assembly of God located at 2640 Parkman Road NW, Warren, OH 44485. This will be an open and walk-in clinic with Pfizer, Moderna, and Johnson & Johnson vaccines available. For those who receive their first dose of the Pfizer or Moderna vaccine, or the single dose of the Johnson & Johnson vaccine at these clinics will be eligible to receive a $100 Gift Card.
To speed up your check-in process we are strongly encouraging everyone to visit our website (www.tcchd.org) to download, print, and complete the COVID-19 Vaccine Administration Record form and bring the completed form along with your driver’s license or one form of identification with you
to the clinic.
Please note that the Pfizer vaccine is eligible for individuals 12 years and older with all minors a parent must be present with them to receive the vaccine. The Moderna and Johnson & Johnson vaccine is only eligible for individuals 18 years and older. TCCHD will NOT provide the vaccine to those that are pregnant or breastfeeding.
