The following are required for enrollment into Warren City Schools. Registration will not be complete until all the items are provided.

  • Birth Certificate
  • Current Immunization Record
  • Social Security Card
  • Parent’s/Guardian’s Photo I.D.
  • Custody Papers (if applicable)
  • Current Proof of Residency in the Warren City School District (Only one required):
    • Utility bill (with top and bottom portion intact) – electric, gas, water (where the service and billing address are the same)
    • Signed lease/rental agreement with contact information for the landlord
    • Monthly mortgage statement in the parent’s name for the given address
    • Aide to Dependent Children (ADC) or Social Security (SSI) benefit letter (current within 2 weeks)
    • Home or auto insurance information for the given address in the parent’s name
    • Work or other income check stubs in the parent’s name with the given address
    • Other records as required by section 3313.64 for other custody or residency situations

Grades K – 8 will be registered at the Administration Offices at 105 High Street, during the hours of 8am to 3pm
Grades 9 – 12 are to be registered at Warren G. Harding High School. Please call 330-841-2316 x2499, for further information.

A child must be 5 years old by September 30 in order to enter Kindergarten.

Registration is available at the Board of Education from 8:30 a.m. – 2:30 p.m. daily.

Parents should bring the following items with them to Kindergarten Registration:

  • CHILD(REN) (Required – as Food Service will enroll child for breakfast/lunch programs)
  • Shot records – updated
    5 doses of D.P.T.
    4 doses of T.O.P.V. (Polio)
    2 doses of M.M.R.
    3 doses of Hepatitis B
    2 dose of Varicella (Chicken Pox)
    *Please note – the State of Ohio requires the above shots before entering Kindergarten.

At registration, parents will receive information on vision and hearing testing for their child, which can either be done at Kindergarten Jump Start (Summer Program) or before school in August.

Jump Start into Kindergarten will be offered to all children enrolled in the Warren City School District who are entering kindergarten. The program is designed to introduce kindergarten children to school routines, as well as to the routines of reading and writing. To register your child, please contact us at the numbers listed above.

  • All students are offered busing to and from the school.
  • Lunch is provided to all students.
  • Jump Start is held at all four of the K-8 Buildings.

Transcripts

To request a copy of your transcripts you have to come into the Student and Community Services office at the Administration Offices or send a letter with your signature stating where you would like the transcripts forwarded to. All requests must include the students date of birth and last name at the time of graduation. The cost is $2 per copy.

If your home is near a boundary line you will need to verify the school associated with your address with the Student Services Department. Regardless of a home’s geographic area, some students may be assigned to a different school based on required programs that are offered at other schools.
